To receive a replacement voter information card you will need to do one of the following:

Be sure to include your name, date of birth and current in-county residence address;
then sign your request.
- or, you may fill out a Florida Voter Registration Application. You can get this application at any of the following places:
Supervisor of Elections offices
Tax Collector Offices
Public service offices
Public Libraries
Offices that serve persons with disabilities
Armed forces recruitment offices
Centers for independent living
Voter registration applications may be picked up but not accepted at most city halls, banks, and community centers. Completed applications can be mailed or delivered in person to any elections office.
NOTE: All registered voters will be mailed new voter information cards prior to the 2012 fall elections after redistricting is completed.
